Directions to Nephrology Academic Offices:

From the University of Washington Medical Center lobby, take the BB elevators to the 12th floor. Our office suite is located on the northwest corner of the 12th floor of the Health Sciences Building.

Training Verification

Submit all training verification payments through our store portal.

  1. Please email Educational Verification forms and signed Authorization of Release forms to Danielle Long, dlong206@uw.edu, after payment has been submitted through the payment portal.
  2. Your email to Danielle should include your full name and your dates of training.
